COVID Vaccination forms for Kids ages 5 -11.
Below are two links for helpful forms to take with you to your child's vaccination. One link is a group of several pages that is all the information from the CDC regarding kids and vaccinations. The Second is a single page to be printed, filled out and brought with you to your child's appointment.

The budget revote will take place on Thursday, June 11th. Absentee ballots will be available beginning May 21st at your Town Clerk’s Office.
