Vermont Agency of Education COVID Survey

The Vermont Agency of Education, in partnership with the Vermont Department of Health, is seeking information to assist with planning a potential COVID-19 vaccine deployment to school staff. The AOE has designed a brief survey to be completed by school staff, in which they will indicate the SU/SD/Independent School where they work, their age range, if they have certain health conditions and whether or not they would be interested to receive the vaccine if it were offered for free in their district or school.


Participation in the survey is voluntary and anonymous. No personally identifiable information (i.e. name, date of birth) will be gathered. The survey is not a registration and completing it does not constitute signing up for or declining a vaccine. This survey is open to all school employees at public and independent schools. Front line contractors (e.g. food service, bus drivers, etc.) will be considered for vaccination, but should not complete the survey at this time.


Please share this survey with your staff and encourage them to complete by 4:30 p.m. on March 1, 2021, at which point the survey will be closed to additional responses.
